For lost or replacement certificates:

We can only issue a replacement certificate to the individual who has taken the course.  We will issue only one replacement certificate per course taken.  Please send the following to the above address:

Check or money order for $5.00 USD for each course for which you need a replacement certificate.

Written request that includes name and address at time course was taken, date of birth, profession, date or approximate date course was taken and a current address to send the replacement. 

Certificate will be mailed after check is honored.  Please allow a minimum of 14 days.
